Fill out, scan and email back to Mr Johnson

Fill out and return the hard copy to Mr Johnson as soon as we are back at school

Please note that when we return to school, and it is safe to do so there will be a combined meeting from Mr Johnson (Gateway) and Mr Barwick (UCOL Trades Academy) to answer any questions that you may have, please ensure you attend that meeting.
Meeting Date is to be confirmed once we are back at school.
